I don't think she'd have let him leave without trying to talk to him again, and it seems so abrupt to have just that one conversation. (Plus the rest of Season 5, but still.) Although she isn't really trying to make him change his mind completely about leaving - I suppose she could always have just ordered him to stay, but the biggest thing always seemed to be that he stayed because he wanted to.
About Lennier's true face: yes, it would be a terrible disappointment for him, whether she was being entirely honest or not. This way he can't console himself with the thought that she'd think differently of him either for better or worse. And whatever he thinks his true face should be, it's probably not the way Delenn's always seen him.
(Maybe that's why they're supposed to watch for three nights, so that the watcher has enough time to get past their own preconceptions? Not that I think Delenn would, really, at this point. I wonder what she saw in Sheridan's face?)
